Tan Kimseng, safety and quality manager at Banyu General Hospital, was found dead in bed 3 of the post-operative recovery room. A man who'd spent thirty years in this hospital, who now graded other people's night shifts with checklists, lay that night in the very room he inspected, after surgery on his own belly. On one wall of that room hangs an automated dispensing cabinet—drawers that open only after identity verification for controlled medications. For separately tracked drugs, two people must verify together before the drawer unlocks; what goes out gets logged in the register and counted at set intervals. That night he received pain medication twice, and by the six a.m. rounds he was already cold. Question the five people who were in that room. Not a single record was altered.
