The Same Number
The chemical plant in the Rinkai industrial zone uses ammonia to cool its processes. In the early hours of early November, Kajita Toshio, the equipment inspection supervisor, was found collapsed in the semi-basement section where the refrigeration system piping runs and was taken to the hospital, but he did not survive the morning. The cause of death was inhalation injury—his throat swelled shut from breathing anhydrous ammonia, pure ammonia without water. At 04:12, liquid sprayed from a pipe that should have been locked off before maintenance. That dawn, the hand that closed that valve and the hand that confirmed it closed were the same.

Exhibits
Number plates hung on corridor-side valve and ladder-top valve
A metal number plate is tied with string below the corridor-side valve handle. The plate is less faded and the string is newer than those on adjacent valves. A chip is attached to the back of the plate, and when the terminal is held to that spot, the screen turns green.
That dawn's permit and terminal confirmation record
The permit for that dawn with the work section and isolation valve number HV-217 written on it. There is a field for each step, and the lockout-tagout, isolation confirmation, and residual pressure confirmation fields are filled in sequence.
That dawn's entry records and night roster
The main gate and piping section door require a card swipe only when entering; when exiting, you push and it opens. Exit times are not recorded. That dawn's roster lists two people for night maintenance and one person for night operations.
Corridor wall drawings and number plate ledger
On the corridor wall of the control room hangs a drawing showing where and how the piping and valves are connected. All valve numbers for this system are listed.
