At 02:38 in the night bakery wing of Fenwick Bakery in the Halbridge Industrial Estate, England, a fire broke out. The responding fire brigade extinguished it in twenty minutes, but during that time flames erupted once outward from the fermentation room doorway, and Maurice Brightwell, the Halbridge Fire Station on-site safety officer standing at that spot, took the blast full in the face and front of the neck. He was carried away without uttering a word and died five hours later in hospital from acute airway burns—his upper airway above the vocal cords swelling shut after inhaling superheated gases. That night there were two ventilation openings to exhaust the smoke, and the order in which the two were opened was the reverse of what the manual prescribed. The five people in front of the bakery wing that dawn each remember a different sequence. Pick out which memory opened that door first.

Fermentation room low fire door
The door is held wide open on its holdback. The paint on the outer face of the door panel has bubbled and burst, and on the threshold in front the victim's unworn mask is lying on the floor.
Roof ventilation skylight and catwalk
Both skylight panels are open and the manual opening lever is pulled all the way. Boot prints remain on the catwalk.
Radio recording and entry log board
That night's radio was recorded in full. A log board for writing by hand the people who entered the smoke and the times is set up beside the pump truck.
Victim's file folder
File folder retrieved from the victim's vehicle passenger seat. Contains several pages of debrief meeting records and hand-drawn opening layout diagrams.
