Consent Form
In a small city upstream, there stands a regional tissue bank that trims and stores tissue excised from human bodies, then ships it to hospitals as needed. The place is designed so that the number assigned on the day of donation follows unbroken until the day of transplantation. Inside the sterile room of that building, Irži Kolář, who had been matching those numbers for twenty-three years, entered one night and came out with a hoarse voice. He washed his eyes at the sink, returned to his desk to finish filling out the cross-reference sheet, and before dawn was carried out gasping for breath. Left in the room were a container with its lid thrown open and an unwashed floor. Five people held positions adjoining that room that night, each pointing to a different box. To know who filled which box, you must ask them. What remains is one box among those meant to be signed by two people side by side—a box no one signed that night.

It is clipped to the clip beside the processing-wing door. Among the six boxes, only the fourth box has both name spaces blank, while the boxes above and below are filled with short marks. The marks are not names but things drawn in two or three strokes.

One day's worth comes out per sheet from the wall panel. There are two lines: one is the air-extraction intensity, the other is the pressure inside the room. Door openings/closings are recorded separately for front and rear doors.

Two waist-high containers are placed, and the lid of the larger one is thrown open to the side. On the floor in front of the container is a wide dried stain. On the shelf by the wall are placed a dry cloth and a powder canister for sprinkling.
the dead man's cross-reference notebook and the documents of the two returned bundles
In a palm-sized notebook, numbers are written in two parallel columns. The left column and right column are aligned at each position, and at the very bottom the handwriting becomes disordered and breaks off. Beside it are placed the past six months' documents.
